The short version: Use this form before importing or analyzing a manuscript. Adapt contract and consent language with qualified professional advice.

Keep intake separate from diagnosis

The intake form captures author goals and operational facts. It should not promise a diagnosis before the editor has read the manuscript or imply that software output determines the edit.

Author and manuscript

Confirm the source material.

  • Author name and preferred contact
  • Manuscript title and version date
  • Genre, audience, and approximate word count
  • Series context or previous books

Goals and concerns

Let the author define the problem in their own terms.

  • What do you want this edit to accomplish?
  • Which parts feel strongest?
  • Which parts concern you most?
  • What feedback have you already received?

Scope and deliverables

Match the form to the contract.

  • Type of edit
  • Material included and excluded
  • Editorial letter, inline notes, call, or other deliverables
  • Deadline and revision schedule

Confidentiality and tools

Obtain specific authorization before processing.

  • NDA or confidentiality requirements
  • Third-party tools permitted
  • AI-assisted analysis permitted: yes / no / discuss
  • Retention and deletion requirements
  • Sensitive or personal data present

Handoff

Prevent version confusion.

  • Preferred file format
